The Good Doctor: Season 4, Episode 20, ‘Vamos,’ Recap & Spoilers

WARNING: The following contains spoilers for The Good Doctor Season 4, Episode 20, “Vamos,” which aired Monday on ABC.

After an eventful start to their surgical trip in Guatemala, the team is put through the wringer in the finale of The Good Doctor Season 4. “Vamos” begins with the surgical process for the chosen few beginning, only for the power to go out soon thereafter. Park quickly comes up with a solution to continue his procedure, but Shaun and Lim don’t have such luck. Thankfully, after talking with Andrews, Shaun realizes he can remove his patient’s tumor without using an ultrasound machine.

Elsewhere, Lea tries to console the new mother she’s been spending time with in the maternity ward because her husband, Oscar, went to care for his sick mother, leaving her without a support system. Shaun continues to operate on his patient, but when he is just about to make a breakthrough, the power comes back, making the surgery much less complicated. The next morning, Lim wakes up next to Rendon, and they talk about their personal lives before embracing.

Back at the hospital, Andrews and Nurse Morales talk to the young boy with a facial embolism about his procedure, with Morales noticing he’s scared about going under anesthesia. Andrews commends his colleague for her bedside manner, but before he can finish, she points out that he’s flirting with her while wearing a wedding ring. Claire talks to her patient’s daughter about her mother’s upcoming procedure, but the girl feels guilty for taking a spot from another worthy candidate.

During Andrews’ procedure, a complication arises that forces Morales and Andrews to leave to try and find a local specialist. They get on the bus to await the driver and begin to discuss the flirting incident, and Andrews reveals his wife is moving out of the house and already seeing someone, which was his reason for coming on the trip. He apologizes for making her uncomfortable, but she tells him it isn’t that; she believes he still wants to save his marriage. However, Morales’ feelings change after Andrews offers his wedding ring to the specialist as a bribe for his help.

Lea is still by the mother’s side after the power comes back on, but after things appeared to calm down, the woman’s baby starts losing oxygen and is put on a ventilator. Having finished his work for the day, Shaun calls Dr. Glassman to ask his advice about Lea moving back home, and despite his own issues, Glassman reassures his friend that he’s going to do the right thing. Andrews finishes his procedure on the young boy with the help of the specialist, and it’s a success, which leaves both him and Morales feeling proud.

Lim once again wakes up next to Morales, and she has a difficult time getting him to open up. He explains that he won’t bear his soul because she’s hiding something, something she adamantly denies. Claire begins her procedure on the mother, only to find an aggressive form of cancer she’s never operated on. Andrews believes they have to cut their losses, but Lim stands behind Claire, and she’s allowed to continue the surgery. Lea goes to check on the mother again and finds she’s without any assistance, so she offers her help for the time being.

Morgan stumbles upon a patient who, after receiving surgery, went back to work and reinjured himself. While she tries to convince Park to take him on, he’s too busy, forcing her to take on the case despite her arthritis. Claire starts her cancer surgery by using a technique she’s never practiced, but with the help of another doctor, things start off on the right foot. Lim tells Rendon during a procedure that she has PTSD but that she wants to start running towards good things in her life rather than away from them.

Morgan beings experiencing pain during her surgery that forces her to stop, but Park arrives in time to take over. Shaun heads to the maternity ward to find Lea helping the baby, and, without saying anything to her, he takes over. Back in Claire’s OR, the surgery is going well until her patient begins crashing for unknown reasons. She goes to Shaun for help, and while she initially takes his opinion as fact, he recalls what Glassman told him and tells her to trust her gut.

Park delivers good news about Morgan’s patient, and the two share a tender moment that’s been a long time coming. Claire saves her patient after Shaun’s pep talk and goes to tell the woman’s daughter, who gives her a massive hug. Meanwhile, in the maternity ward, Shaun does another check-up on the baby, and after giving it a clean bill of health, Lea holds her for the first time.

Having finished their mission, the team says their goodbyes to the people at the hospital and heads onto the bus. On the trip to the airport, some major revelations take place: Park and Morgan admit they love each other, Rendon explains he can’t return to the United States because of an active warrant but wants to go back to see Lim and one of the local doctor wants to ask Claire an important question. She offers Claire a senior position at the hospital, and while Claire has reservations, Lim convinces her friend to take it.

After hugging his dear friend Claire for the first time, Shaun sits on the airport bench with Lea as she tells him she’s not leaving for Pennsylvania. Believing she’s better off with her boyfriend in her life, Lea proposes to Shaun, and the entire team embraces as Claire begins a new chapter in her life.
